    When to use Remdesivir, Ivermectin? India’s Covid treatment protocol and what experts say

    The Union health ministry has issued clinical guidance for the management of Covid-19 patients, underlying which medication is to be administered and at what stage. The usage of Remdesivir, Ivermectin for treatment has, however, come under heavy criticism amidst warnings by WHO.

    At present, none of the drugs being used in India for the treatment of disease is new. Instead, these are repurposed drugs — initially developed for treating a different infection and now repurposed to manage patients of Covid-19.

    The Union health ministry had last month issued clinical guidelines for the management of Covid-19 patients, underlying which patients need which medication and what should not be done in a specific situation.

    While this treatment protocol for Covid patients — mild, moderate, and severe — is to be followed only by doctors, we take a look at what happens in the three stages of infection and which drug is to be administered, and when.

    A look at India’s clinical management protocol:
    Covid treatment protocol for people with mild disease
    In the case of mild infection, upper respiratory tract symptoms like fever without shortness of breath are seen. In such cases, medical experts suggest home isolation and care at home.

    While at home, physical distancing, indoor mask use, and hand hygiene are important. Hydration, antitussives, and multivitamins are to be given to the patient but only in consultation with a doctor. Monitoring of oxygen and temperature is also required.

    Experts say immediate medical attention must be sought if there is difficulty in breathing, high-grade fever, cough, or if symptoms are persistent beyond five days of the onset of the disease.

    >> Medicines: Therapies based on low certainty of evidence are Ivermectin, which doctors say must be avoided by lactating and pregnant women. An alternative to Ivermectin is Tab HCQ unless contraindicated, and inhalational budesonide.

    Ivermectin is a medicine used to treat parasite infestations. In humans, this includes head lice, scabies, river blindness. Meanwhile, HCQ is a medicine to treat and prevent malaria. Other uses include the treatment of rheumatoid arthritis and lupus.

    Budesonide in the inhaled form is used in the long-term management of asthma and chronic obstructive pulmonary disease.

    For moderate case of Covid-19
    In cases of moderate disease, the respiratory rate is more than 24 per minute. Patients experience breathlessness as the oxygen saturation rate plummets to 93.

    In such cases, the government has advised admitting the patient to a Covid care facility. Target SpO2 and ensure saturation between 92 to 96 per cent, experts say.

    The preferred device for oxygenation is a non-rebreather face mask in such cases, and awake proning is advised. Anti-inflammatory or immunomodulatory therapy is also advised.

    >>Medicines: Injectable Methylprednisolone is to be prescribed or an equivalent dose of dexamethasone. Anticoagulation, which is a conventional dose of unfractionated heparin, or low molecular weight heparin is advised.

    Clinical monitoring of breathing, hemodynamic instability, or change in oxygen requirement is to be done.

    HRCT chest is to be done only if the condition worsens. Lab monitoring of CRP and D-Dimer is to be done on a 48-72 hourly basis. Tests like CBC, LFT, KFT are to be done, and IL-6 if the levels worsen.

    Dexamethasone is a glucocorticoid medication a steroid used to treat rheumatic problems, a number of skin diseases, severe allergies, asthma, chronic obstructive lung disease, croup, brain swelling, eye pain following eye surgery, along with antibiotics in tuberculosis.

    Medications like Remdesevir and Tocilizumab and convalescent plasma can be explored too.

    Let’s see what these drugs are:

    Remdesivir, made to treat Ebola, is a broad-spectrum antiviral medication.

    Tocilizumab, also known as Atlizumab, is an immunosuppressive drug, mainly used for the treatment of rheumatoid arthritis.

    Convalescent plasma therapy is using blood from people who have recovered from an illness to help others recover. This is used for other diseases as well and has to be done within seven days of the onset of the disease.

    For serious case of Covid-19
    A moderate case of Covid-19 becomes severe when the patient’s respiratory rate is more than 30 and oxygen saturation less than 90.

    Here, the use of NIV is advised in patients with oxygen requirements. The use of HFNC is also suggested. In such cases, intubation should be prioritised in patients and use conventional ARDSnet protocol for ventilator management.

    >> Medicines: Dexamethasone and use of heparin are advised. In serious cases too, HRCT, CRP, D-Dimer and other blood tests are prescribed.

    The USA is on the cusp. Representative Ocasio-Cortez calling Representative Greene “deeply unwell” is signal. So is January 6th, the DarkSide Hack, 737 Max, PG&E wildfires, the Texas Freeze, and the auto assembly shutdown due to computer chip shortage.

    The disdain of the globalist top 10% for lower class Americans is shown by the US federal government’s ending of masking and social distancing for more than half of all Americans who are unvaccinated in the middle of a pandemic. Masks mostly protect one from getting droplets sprayed on them by super-spreaders. Now no one will wear a mask in public for fear of the jeers and violence. With the virus endemic in the USA and unhindered air transportation around the world; Indian, European and American variants will spread worldwide. A winter spike will rip the USA apart.

    China, Vietnam, South Korea, Singapore, Thailand, Iceland, Australia and New Zealand, isolated from the rest of the world, will try to keep human civilization going in a New Dark Age.

    You left out Russia; for what reason? Russia is the most massive country geographically, and probably possess’ the largest untapped resources on the planet.
    Further, Russia would likely survive a nuclear exchange better than any other; they’re probably the most capable country with their advanced anti-missile systems…judged to be the most effective in real world scenarios.
    I’m not that optimistic about SE Asia for numerous reasons; rule by the military for one (Myanmar, Lao, Kampuchea, Thailand, and Vietnam)…
    I’ve been here through 2 coups…so far so good…(that’s not an endorsement… 😉

    I am not a Russian expert and I never have been to Europe, Africa or South America. Russia is #7 on the list of nations for coronavirus deaths. I attribute this to the infestation of neoliberalism introduced by Boris Yeltsin and the Harvard Boys. Vladimir Putin has reined in the oligarchs to a degree but not completely.

    Democracy has two stellar advantages 1) the peaceful transfer of power and 2) consent of the governed. Since January 6th the USA has lost both. Russia is in a hard place between Europe and China. The corporate western empire is pursuing a regime change campaign against the Kremlin. A peaceful transfer of power would be a triumph for the Russian people.

    I visited Thailand over a half a century ago; Hat Yai, Bangkok and Chiang Mai. Between Burma and Vietnam with China not far away and Malaysia to the South, it is on the fault line in the clash of civilizations. Never being a colony indicates that the Thais know this. Hopefully they will stay united and the military doesn’t usurp the kingdom.

Hopefully they will stay united and the military doesn’t usurp the kingdom.

    Yes, that is a key point; never having been colonised…
    The monarchy is still strong here; the military being subservient…so far…

    As to Russia; one just needs to look at its string of firsts in the realms of space flight/science/ and medicine…
    The only thing lacking for Russia is robust population numbers…
